Terms and Conditions for Appointments and Packages

Patients must meet medical guidelines and be approved by NP/MD for Phentermine prescription.

Expiration Dates

• 3-month packages must be used within 6 months of purchase date
• 6-month packages must be used within 12 months of purchase date

Refund Policy

• A $10 administrative cancellation fee will be charged for all refunds
• For any package not fully used, a refund may be provided after deducting the full price (i.e. non-discounted fee) of services received.
  Example: A returning patient purchased a 3-month package for $297 and used only one of three visits. From the $297 paid, we’ll deduct $110 for one full-price visit and the $10 administrative cancellation fee, then issue a $177 refund.

No-Show Fee: A $25 fee will be charged for any missed appointment or last-minute cancellation.
